If you are having technical problems completing this survey, please email the following information to

3) The airport where you received your Invitation Card.

4) The serial number you were given on your Invitation Card.

5) The date and time when you encountered the problem.

6) The nature of the problem you encountered and where in the survey you encountered it.

7) If an Error message was displayed and is still available, please copy and paste it into your message. If it is not available, please try to recall as much of what it said as you can.
